Along with help understanding both state provided documents, GCMC community relations director Carla Offenburger says they also provide information on the Iowa Physician Orders of Scope Treatment forms.  She comments that this is a separate form from the Living Will and Durable Power of Attorney in that it compiles and summarizes a patient’s wishes for key life-sustaining procedures.

Offenburger asks all GCMC patients to fill out all documents or update them so that your physician knows what to do if you are unable to make decisions about your health care needs.
