The Perry Rotary Club is looking for nominations for the 2012 Outstanding Civil Servant award.
Individuals who work or volunteer in law enforcement, the fire department, ambulance squad, emergency services, health care or human services are eligible for nomination.
Nomination forms can be found at the Perry Public Library, Perry Chamber office, McCreary Community Building at the Finneseth, Dalen and Powell PLC office building.
Nominations should be sent to the Finneseth, Dalen and Powell office at 1401 Willis Avenue or P.O. Box 487 in Perry. Forms must be turned in my 3pm Friday October 5th.
